PPE Product Complaint Form

We have implemented a process to receive, evaluate, investigate and report any complaints received in regards to Personal Protective Equipment (PPE) distributed by the Government of Ontario.

The sourcing of pandemic PPE is conducted by multiple organizations, primarily Ontario Health, the Ministry of Health, the Ministry of Government and Consumer Services and the Federal Government.
